HEALTH QUOTIENT (REVISED)
For Active Participants in the Conference Health Plan
For all the clergy and spouses who are currently participating in the Conference Health Active Insurance Program (HealthFlex), please remember to take your yearly Health Quotient (HQ) assessment by September 30, 2016. The new HQ is quick - 15 minutes or less - even faster if you took the Blueprint for Wellness screening this year. It can also be completed on a mobile device connected to the internet. Plus, the assessment helps to avoid paying extra on your 2017 medical plan deductible.
Please note, this assessment is provided as part of your wellness benefits. The assessment is totally separate from your health insurance and plays no part in your benefit determination or rates. It is solely for your information to help pinpoint areas that may impact your health either now or in the future. Individual information is not shared with Blue Cross/Blue Shield or anyone within the conference.
Completing the HQ by September 30 - avoid paying extra on your 2017 medical plan deductible:
$250 extra for individual deductible
$500 extra for family deductible
* Please note both spouses must participate in order to receive the reduced deductible for a family.
If you (and your spouse) have already completed your HQ assessment between August 1 and now, thank-you! You do not need to do anything further. For those of you who have not completed the assessment, please log into WebMD to complete this task.

There is now an updated form 8 available in the Charge Conference forms. Please note, if you have already filled out your form 8, it is acceptable. You do not have to do it again on the newer form.

SEPTEMBER 1 DEADLINE FOR HEALTH MINISTRIES GRANT APPLICATION
Is your church planning health-related activities for 2017? The Mission and Outreach Team has funds to assist with new health-related programs and with existing programs that demonstrate success. Preference will be given to projects that fulfill our mission of making disciples of Jesus Christ for the transformation of the world. Innovative programs that meet real community needs in connection with other local ministries and other local churches will be given high priority.
Grants may be given for parish nurse related programs, including initial training for parish nurses and/or health ministries, for purchase of blood pressure apparatus and first aid kits, for printed materials needed to begin a program, expenses for a health fair, and toward the purchase of an AED and CPR/AED training up to designated amounts.
The deadline for 2017 funds is September 1.
